Selected apical reconstruction
A programme series studied transapical myectomy together with apical aneurysm repair in 67 patients treated during 2000–2020.
Apical aneurysm repair addresses a selected structural problem. This evidence does not support surgery for every patient with apical or nonobstructive HCM and is not an individual surgeon score.
Joseph Dearani’s Mayo practice spans HCM, Ebstein anomaly, congenital surgery and complex reoperations. Mayo’s selected apical-myectomy research addresses a different problem from routine basal septal obstruction.
The Mayo series covers transapical myectomy plus apical aneurysm repair between 2000 and 2020. Median follow-up was 4.9 years; reported estimated five-year survival was 94.5%.
How to interpret this: This selected retrospective programme cohort is not an individual surgeon score. It does not establish that all apical or nonobstructive HCM requires surgery.
