Yong Cui: septal myectomy and minimally invasive HCM surgery

HCM surgery addressing the septum and mitral supporting structures · Hangzhou, China

At a glance

Yong Cui’s team has published HCM surgery through an incision near the right armpit, leaving the breastbone intact. The newer 100-patient MESM series reported a substantial reduction in outflow obstruction; the earlier 148-patient series also describes correction of abnormal structures supporting the mitral valve.

Relief of outflow obstruction

In the 100-patient MESM study, the mean pressure gradient across the narrowed outflow fell from 85.8 to 8.4 mmHg. These were early postoperative results.

4–5 cm incision; breastbone intact

The study used an incision near the right armpit. No patient in that series required conversion to sternotomy.

Procedure-specific research

Right infra-axillary transaortic myectomy with documented subvalvular work.

148 patients. September 2021–July 2023 · published 2024. Papillary resection in 72/148 (48.6%); chordal resection in 93/148 (62.8%); papillary reorientation in 15/148 (10.1%). Procedures overlap. Numerical follow-up duration was not specified in the inspected report.

How to interpret this: Cui is senior/corresponding author; the full paper does not explicitly assign all operations to him personally. Selected single-centre approach series without a sternotomy control group. Pacemaker risk is affected by baseline conduction abnormalities. This is the earlier modified-Morrow series, not the newer 100-patient MESM series.

Electro-septal sculpting through a 4–5 cm right infra-axillary incision.

100 patients. November 2023–May 2024 · print publication 2026. A distinct newer technique cohort. The practice site describes a long electrocautery instrument; the study abstract reports aortic cross-clamping. Early postoperative outcomes; long-term duration unavailable in the abstract.

How to interpret this: Cui is senior/corresponding author and the practice site’s technique proponent. Personal operator count is not established by the abstract. Abstract-only review. Do not sum this cohort with other publications or the practice website’s lifetime claims. Zero observed deaths does not mean zero risk.

The Hangzhou team has published operations that relieve obstructive HCM through an incision near the right armpit. Its newer 100-patient MESM study reports early relief of obstruction while preserving the breastbone. An earlier study also treated abnormal mitral-valve supports that contributed to the blockage.
