Paolo Ferrazzi: HCM myectomy and mitral chordal cutting

HCM myectomy with correction of abnormal secondary chordae · Monza, Italy

At a glance

Paolo Ferrazzi’s Monza work treats obstructive HCM by removing excess heart muscle and selectively cutting abnormal cords that pull on the mitral valve. The aim is to open the outflow pathway while keeping the patient’s own valve. The published 350-patient study has a median 5.6 years of follow-up.

Anatomy-specific valve preservation

The technique targets abnormal secondary chordae contributing to obstruction, rather than routinely cutting normal valve supports.

350-patient long-term cohort

The 2026 report includes 350 consecutive patients, with median follow-up of 5.6 years and follow-up available for 344.

Reported symptom status

At latest follow-up, 271 patients were in NYHA class I and 65 in class II—little or mild limitation of activity.

Procedure-specific research

350 patients, followed for a median 5.6 years

The 2026 JACC report studied 350 consecutive patients treated at one centre over a five-year operating period. Follow-up was available for 344 patients. At the latest assessment, 271 were in NYHA class I and 65 in class II.

How to interpret this: These are centre-cohort results, not Ferrazzi’s independently audited personal total. This observational study did not compare this technique directly with Cui’s MESM or other European surgeons.

Clinical programme

Policlinico di Monza →

Monza, Italy

Monza’s HCM work combines muscle resection with correction of selected abnormal secondary mitral chordae. The purpose is anatomical relief of obstruction while retaining the native valve, with longer follow-up than a purely early feasibility report.
