How surgery works and how approaches differ

Hysterectomy removes the uterus. The choice involves both what tissue is removed and how the surgeon operates. For non-cancerous uterine disease, total hysterectomy removes the cervix as well, while subtotal hysterectomy keeps it. A 2021 guideline recommends removing the cervix when hysterectomy is performed for benign uterine disease.

For early-stage cervical cancer, radical hysterectomy raises a different question: how to treat the cancer while protecting nearby pelvic nerves. Nerve-sparing surgery aims to protect bladder, bowel and sexual function. Open and minimally invasive radical hysterectomy also need to be considered separately: evidence about surgery for benign disease should not determine the cancer operation.

When surgery may help and what alternatives to discuss

For early-stage cervical cancer, discuss the cancer-survival evidence and the treating centre’s own results before deciding on minimally invasive radical hysterectomy. A 2019 position statement recommends that this approach be reserved for carefully selected patients and performed by gynecologic oncologists skilled in the technique.

For postmenopausal women having sacral colpopexy to repair pelvic organ prolapse, the decision is different. A 2021 guideline advises against adding subtotal hysterectomy solely to prevent endometrial cancer: the additional operation has risks of its own. This recommendation concerns prolapse surgery, not treatment of an already diagnosed cancer.

Expected outcomes and limits of the evidence

Results depend on the reason for surgery. In a 2026 review of 14 randomized trials involving 1,921 women with benign gynecological disease, total hysterectomy reduced abnormal or cyclical vaginal bleeding at one year compared with subtotal hysterectomy. The relative risk was 0.15, with a 95% confidence interval of 0.07 to 0.33; this is a comparison between groups, not a 15% chance of bleeding. Urinary incontinence was less frequent after total hysterectomy in analyses extending beyond one year, up to 14 years, but this difference was not seen at one year or in a sensitivity analysis extending to two years. Other urinary, sexual and bowel function measures and overall quality of life did not differ significantly.

For cervical cancer, a 2019 position statement reports that a randomized trial found lower disease-free and overall survival after minimally invasive radical hysterectomy than after open surgery. A 2026 review of 15 studies also favoured open surgery for five-year disease-free survival, but found no statistically significant overall-survival difference. These findings do not establish that the approaches are equivalent or provide an individual patient’s chance of cure.

Risks to discuss with your surgeon

For benign uterine disease, keeping the cervix is associated with a greater risk of needing another operation for cervical bleeding. For early-stage cervical cancer, radical hysterectomy can damage pelvic nerves and cause problems with urination, bowel movements or sexual function.

A 2026 review of 10 trials involving 1,247 patients undergoing open abdominal radical hysterectomy for cervical cancer estimated bladder injury during surgery at 0.2% and injury to a ureter, a tube carrying urine from a kidney to the bladder, at 0.3%. The respective 95% confidence intervals were 0% to 1.2% and 0% to 1.1%. Later urinary problems included leakage through abnormal connections, obstruction and difficulty emptying the bladder. Their reported frequencies varied substantially with definitions and assessment methods, so they should not be treated as a single precise risk estimate.

Recovery and follow-up care

Going home on the day of robotic hysterectomy is a possibility for selected patients, not an automatic part of the operation. A 2026 review included 3,200 patients in 10 cohort studies: 1,801 went home the same day and 1,399 stayed in hospital. Thirty-day readmissions did not differ significantly between the groups. Because discharge decisions selected patients with different health and surgical circumstances, this does not prove equal safety for everyone. Older age, obesity, other illnesses, anticoagulant use, anaemia and more extensive surgery were among the factors associated with a lower likelihood of same-day discharge.

After hysterectomy for cervical precancer or cervical cancer, abnormal cells can still develop in the vagina. Discuss follow-up with the treating team, including assessment of bladder, bowel and sexual function after radical surgery.
