How surgery works and how approaches differ
For early-stage endometrial cancer, laparoscopic hysterectomy has been studied as an alternative to abdominal hysterectomy. The comparison concerns total laparoscopic hysterectomy, rather than the modified radical operations studied for cervical cancer. Within laparoscopic surgery for endometrial cancer, single-port and multi-port approaches are two variants that have been compared.

When surgery may help and what alternatives to discuss
Early-stage endometrial cancer is one setting in which evidence supports laparoscopic hysterectomy compared with abdominal surgery. These findings apply to that cancer and stage, rather than to every reason for having a hysterectomy. Single-port and multi-port laparoscopic surgery have also been compared in patients with endometrial cancer; this is a choice between laparoscopic techniques, not a comparison with treatment that avoids surgery.

Expected outcomes and limits of the evidence
For early-stage endometrial cancer, a 2018 analysis included nine studies with 4,405 patients overall. Five studies reported disease-free survival, meaning survival without the cancer returning. At three years, the estimated difference in disease-free survival was 1.44 percentage points in favor of abdominal rather than laparoscopic hysterectomy. The uncertainty interval ranged from 0.65 percentage points in favor of laparoscopy to 3.53 percentage points in favor of abdominal surgery. This was within the analysis’s prespecified five-percentage-point limit for judging laparoscopy not substantially worse, rather than proof that the operations give identical results.
Risks to discuss with your surgeon
Nausea and vomiting after laparoscopic hysterectomy can be associated with opioid pain medicines. A 2025 analysis of 17 studies involving 1,315 participants found no statistically significant difference in this risk between six opioids used individually. Some combinations that reduced reliance on opioids had a lower risk than an oxycodone-based combination. Shoulder pain can also occur after total laparoscopic hysterectomy. In a 2024 trial of 75 women having uncomplicated surgery and no previous shoulder pain, a shoulder-movement routine did not reduce shoulder pain significantly compared with a hand-movement routine at six hours, 24 hours or seven days.
Recovery and follow-up care
Early recovery includes pain control and the return of bowel function. A 2022 trial enrolled 135 patients having laparoscopic hysterectomy and compared three opioid medicines delivered through a patient-controlled intravenous pump. Average time to first passing gas was 45.2 hours with butorphanol, 33.1 hours with fentanyl and 36.2 hours with oxycodone. Butorphanol was also associated with more sleepiness and dizziness. The groups did not differ significantly in time to tolerate food by mouth, first walking or postoperative hospital stay. These are results from a particular pain-treatment trial, not a timetable for recovery after cancer surgery.
