Hysterectomy with DaVinci robot

Robot-assisted hysterectomy is a form of laparoscopic surgery to remove the uterus. The da Vinci system is one robotic platform used for this operation. Open surgery and conventional laparoscopy are other surgical approaches; the route used and the extent of surgery are separate choices.

Robot-assisted hysterectomy has been studied for endometrial cancer, which affects the lining of the uterus, including early-stage disease. Comparisons with conventional laparoscopy and open surgery address different aspects of the choice, such as blood loss and hospital stay. In patients with endometrial cancer, the size of the uterus relative to the pelvis has been linked to operating time. These findings do not establish which approach is best for an individual patient or justify applying endometrial-cancer results to cervical cancer.

In a comparison published in 2026 involving 300 patients with endometrial cancer treated with da Vinci surgery and 80 treated with conventional laparoscopy, robotic surgery was associated with less blood loss, while laparoscopy took less time. These are findings about the operation, not long-term cancer control. For women with endometrial cancer, evidence published in 2017 also linked robotic surgery to less blood loss than either conventional laparoscopy or open surgery, but the evidence was rated very low quality.

The complication comparisons differ: the 2026 comparison of 300 da Vinci operations and 80 conventional laparoscopic operations for endometrial cancer reported fewer complications with robotic surgery, while the 2017 evidence found no difference in overall complications between robotic surgery and conventional laparoscopy. Breathing during the operation is another consideration. In 27 patients with endometrial cancer or atypical changes in the uterine lining who had a body mass index of at least 25, oxygenation was maintained during da Vinci Xi surgery with controlled ventilation; patients with a body mass index of at least 40 required higher airway pressures.

For women with endometrial cancer, evidence published in 2017 linked robot-assisted surgery to a shorter hospital stay than open surgery, but not a shorter stay than conventional laparoscopy.
