How surgery works and how approaches differ

For a Whipple operation, also called pancreaticoduodenectomy, laparoscopic surgery is a minimally invasive approach; robotic surgery is another approach alongside open surgery. Choosing between these approaches involves matching the complexity of the operation to the surgeon’s and hospital’s experience. For liver cancer, liver resection is an operation to remove cancerous liver tissue. Minimally invasive liver resection requires specific technical skills, particularly when surgery follows the liver’s anatomical divisions.

When surgery may help and what alternatives to discuss

When considering a robotic or laparoscopic Whipple operation, the assessment includes the size of the pancreatic and bile ducts, whether the tumour touches blood vessels, obesity, and previous complicated upper abdominal surgery or disease. These factors are considered alongside the team’s experience. If the tumour involves the portal or superior mesenteric vein, removing and reconstructing the affected vein may allow surgery in selected patients; this is a technically demanding operation with complex care needs. For liver surgery, planning care takes account of the disease, the patient’s health and how well the liver works.

Expected outcomes and limits of the evidence

When discussing pancreatic or liver surgery, ask the team how it measures results. Relevant measures include major complications, bile leaks, unplanned further procedures within 90 days, and deaths within 90 days of surgery. These address different aspects of safety, rather than a single overall success rate. For robotic or laparoscopic Whipple surgery, a comparison between centres should also consider the complexity of the patients’ operations and the experience of each team.

Risks to discuss with your surgeon

Whipple surgery is a complex operation with substantial risks of complications and a risk of death around the time of surgery. Before this operation, identifying narrowing of the coeliac artery, a blood vessel supplying abdominal organs, matters because it can increase the risk of inadequate blood flow after surgery. After left, or distal, pancreatectomy, one concern is a pancreatic fistula, meaning leakage of pancreatic fluid. After liver resection, important complications include bile leakage, bleeding and liver failure. The liver’s function, the disease and the patient’s health all contribute to risk.

Recovery and follow-up care

Recovery planning for a Whipple operation involves several healthcare professionals working together. An enhanced recovery plan includes explaining the process to you, planning nutrition, using a combination of pain-relief methods, managing fluids and blood sugar, and helping you restart eating and moving early after surgery. Drains may be removed early when clinically appropriate. For liver resection, preparation to improve fitness before surgery and early movement afterwards are recommended parts of care. Nutrition is also part of planning care around the operation. These are useful topics to discuss with the team when preparing for treatment.
