A distal pancreatectomy, also called a left pancreatectomy, removes the body and tail of the pancreas.

A distal pancreatectomy, also called a left pancreatectomy, removes the body and tail of the pancreas. The cut end of the remaining pancreas is closed, for example with stitches or surgical staples. Neither closure technique has shown a clear advantage in preventing pancreatic leakage. This operation can be performed through an open incision or with minimally invasive techniques, including laparoscopic and robot-assisted surgery. These are different ways of performing the operation, not different guarantees of cancer control.
For pancreatic ductal adenocarcinoma, surgical removal offers a possibility of cure. When a distal pancreatectomy is being considered, previous pancreatic surgery, inflammation of the pancreas, the tumour’s position and involvement of nearby blood vessels can make the operation more difficult. These factors matter when planning the surgical approach.
Open and minimally invasive distal pancreatectomy have been compared specifically in people with pancreatic ductal adenocarcinoma. A separate question is whether the spleen can be preserved in people with small pancreatic neuroendocrine tumours, a different tumour type. Keeping the spleen is a potential option for selected patients with small, well-differentiated grade 1 or 2 neuroendocrine tumours, but the supporting comparisons are preliminary and do not establish it as an alternative for ductal adenocarcinoma.
In comparisons involving 9,339 people with pancreatic ductal adenocarcinoma across 20 studies, 2,219 had minimally invasive distal pancreatectomy and 7,120 had open surgery. The pooled relative risk of death within 90 days was 0.43 for minimally invasive surgery compared with open surgery; this is a relative comparison, not an individual patient’s percentage risk. Infections and blood transfusions were also less frequent with minimally invasive surgery, while pancreatic fistula, overall complication grades and repeat operations did not differ significantly. The evidence combined randomised trials with observational studies, so these associations do not establish that changing the surgical approach alone produces the same benefit for every patient or improves long-term survival.
An important complication after distal pancreatectomy is a pancreatic fistula: leakage from the remaining pancreas after surgery. Fluid collections can also develop after left pancreatectomy. Some are visible on scans without causing symptoms; others cause symptoms, become infected or need an intervention. Studies assessing these collections within 90 days used differing definitions, making their results difficult to compare. Stitches and staples have not shown a clear difference in pancreatic fistula risk, and choosing a minimally invasive approach does not remove that risk.
Among 9,339 people with pancreatic ductal adenocarcinoma in 20 studies comparing distal pancreatectomy approaches, the pooled hospital stay was 1.99 days shorter after minimally invasive surgery than after open surgery. This describes the difference during the surgical admission, not the total length of stay or the time needed to recover at home.
When discussing care with a hospital, useful questions include whether a specialist pancreatic surgical team and supporting services are available, and how the hospital tracks serious complications, unplanned interventions and deaths during the first 90 days after surgery. These are recognised measures of surgical care quality, rather than a schedule for cancer follow-up.
