How surgery works and how approaches differ

Breast reconstruction after mastectomy can use your own tissue, an implant, or a combination of both. An implant may be placed in one operation or in stages, first using an adjustable tissue expander and later replacing it with a full-volume implant. Mastectomy can also be performed without reconstruction.

For breast cancer that has not spread to distant parts of the body, reconstruction may be immediate or delayed. The choice depends on your preferences, other health conditions, the type of mastectomy, blood supply to the skin, and your existing and desired breast size. These are decisions to discuss with your surgeons rather than a single approach that fits everyone.

  • A meta-analysis of 17 studies involving 399 patients and 401 lesions smaller than 2 cm reported 99% technical success, 96% complete ablation, a mean ablation time of 15.8 minutes, and a pooled complication rate of 2%; however, most studies were small, and 65.7% of patients underwent post-ablation surgical excision. Study follow-up: More recently, the multicenter, single-arm phase 3 RAFAELO study treated 370 patients with Tis-T1 tumors measuring 1.5 cm or less and reported 98.6% 5-year ipsilateral breast tumor recurrence-free survival among 353 patients completing follow-up, with two ipsilateral recurrences.; Radiofrequency ablation (RFA) is a minimally invasive local treatment option for selected patients with early breast cancer, with potential advantages in cosmetic preservation and reduced physical burden. When surgery may help and what alternatives to discuss

If you are considering keeping the nipple during mastectomy, eligibility needs careful assessment. For people with non-metastatic breast cancer who are suitable for skin-sparing mastectomy, nipple preservation is recommended when examination, imaging and tissue assessment show no nipple or areola involvement, the operation is technically feasible, and an acceptable appearance can be achieved.

Cancer in one breast does not automatically mean the other breast needs to be removed. Preventive removal of the opposite breast is not recommended for women with cancer in one breast who have no family history or genetic predisposition. A separate assessment is needed for higher-risk situations: a 2018 consensus recommended this option for women with cancer in one breast and a BRCA1 or BRCA2 gene mutation.

Mastectomy does not necessarily remove the need for radiotherapy. After surgery as the first treatment, radiation is recommended for most patients whose cancer involves lymph nodes and for selected patients without lymph-node involvement. After drug treatment before surgery, it is also recommended for patients who initially had locally advanced disease or still have cancer in lymph nodes at surgery.

Expected outcomes and limits of the evidence

One finding concerns hidden cancer discovered when the opposite breast is removed preventively, not the chance of cure after treatment. A 2026 review of 27 retrospective studies included 5,728 patients undergoing this operation. Previously unsuspected invasive cancer was found in 87 patients at the time of surgery; the pooled estimate across studies was 1.55%. This is a finding at surgery, not a long-term recurrence rate or a comparison showing that preventive removal improves survival.

Risks to discuss with your surgeon

The risks depend on the operation and reconstruction plan. In nipple-sparing mastectomy and reconstruction, preserving blood supply helps reduce tissue death, while surgical planning also aims to limit nerve damage. When a supporting tissue matrix is used around an implant, infection and fluid collection are concerns; it should not be used to compensate for skin with blood supply too poor for an implant above the chest muscle.

For breast-conserving surgery, adding reshaping techniques involves a trade-off. A 2017 expert consensus considered these techniques useful for appearance and for extending breast conservation to some larger tumours, but agreed that complications were more likely than with conventional breast-conserving surgery. The approach needs to be tailored to the individual patient.

Recovery and follow-up care

Planning for life after surgery includes deciding whether reconstruction will be immediate, delayed or staged. With staged implant reconstruction, the expander is used first to stretch the tissues while protecting their blood supply, and a full-volume implant is placed later. This means planning for more than one procedure.

If you are considering preventive breast removal, preparation can also include a psychological consultation. Topics include your understanding of cancer risk and the operation, its possible physical and emotional effects, informed decision-making, anxiety and body image.
