How surgery works and how approaches differ
Breast-conserving surgery aims to treat the cancer while keeping the breast. An oncoplastic approach combines breast-conserving surgery with reshaping. It can allow larger amounts of tissue to be removed while preserving breast shape. The operation needs to be tailored to the individual rather than treated as one standard technique.
When surgery may help and what alternatives to discuss
For some people with larger tumours, oncoplastic surgery can extend the possibility of breast conservation. Having more than one cancer in the same breast does not automatically rule it out: breast conservation has been compared with mastectomy in this setting, with the important conditions that the edges of the removed tissue are free of cancer and radiotherapy is given afterwards. Differences between the patients selected for each operation limit how confidently these comparisons can guide an individual choice.
Treatment of the underarm lymph nodes is a separate decision. In selected early-stage breast cancers with cancer in one or two sentinel nodes—the nodes sampled to assess spread—studies have compared limited node surgery, sometimes with underarm radiotherapy, against more extensive node removal. These findings concern limited node involvement, not every pattern of cancer spread.

Expected outcomes and limits of the evidence
The extent of lymph-node surgery can affect swelling risk without necessarily improving cancer outcomes. A 2025 review of seven randomised trials included 7,338 women with breast cancer and one or two affected sentinel nodes, followed for 2.8 to 10 years. Limited sentinel-node surgery was associated with a 65% lower relative risk of lymphoedema, or swelling caused by impaired lymph drainage, compared with more extensive underarm node removal. This is a relative reduction, not 65 fewer cases in every 100 women. The review found no statistically significant differences in survival, survival without disease, or recurrence at the reported 5-, 8- and 10-year assessments. These results apply to the selected node-treatment settings studied, including some use of underarm radiotherapy; they are not a survival estimate for all breast cancer operations.
Risks to discuss with your surgeon
Possible problems after breast cancer surgery include bleeding, persistent pain, changes in sensation and lymphoedema. Risk depends in part on the operation: more extensive underarm node removal carries more lymphoedema risk than limited sentinel-node surgery in the patients studied. Oncoplastic breast conservation also involves a trade-off: a 2017 expert consensus supported its potential to improve breast appearance and treat larger tumours, but agreed that complications are more likely than with conventional breast conservation.
Preserving the intercostobrachial nerve, which contributes to sensation around the underarm, probably reduces persistent postsurgical pain and sensory problems compared with dividing it. Evidence about how nerve preservation affects cancer recurrence is much less certain.

Recovery and follow-up care
Pain control is an important part of early recovery. Regional nerve blocks use local anaesthetic to numb a particular area. A 2025 review included 39 randomised trials with 2,348 women undergoing breast cancer surgery; most trials involved modified radical mastectomy. At 24 hours, the comparison of pain at rest drew on 13 trials with 793 participants. Pectoral nerve blocks reduced pain by an average of 0.32 points on a 0–10 scale compared with paravertebral blocks, less than the one-point difference the review considered clinically important. This comparison concerns early pain relief, not how soon someone can return to work or usual activities.
