How surgery works and how approaches differ
Gastrectomy is surgery to remove part or all of the stomach. For stomach cancer, the operation also involves rebuilding the route through which food passes. The amount of stomach removed depends on the cancer’s location and stage. Two distinct choices are how much to remove and how to reconnect the digestive tract. When part of the stomach can be kept, reconstruction aims to preserve its function as well as treat the cancer.
When surgery may help and what alternatives to discuss
For early stomach cancer, function-preserving treatment aims to remove the cancer completely while limiting the extent of surgery and keeping as much stomach function as possible. This approach includes more limited removal and, in selected situations, endoscopic resection, which removes tissue using an instrument passed into the stomach. Preserving stomach function must not come at the expense of cancer treatment. Total gastrectomy, which removes the whole stomach, is another approach used with the aim of curing stomach cancer; a limited operation is not interchangeable with it for every patient.
Expected outcomes and limits of the evidence
Treatment has two linked goals: removing the cancer and protecting daily life after surgery. With function-preserving surgery, assessment includes how well the remaining stomach works, nutrition, complications affecting digestion and quality of life. After removal of the upper part of the stomach, the way the digestive tract is reconnected can help reduce reflux, when digestive contents flow back into the food pipe. Preserving part of the stomach does not by itself prevent reflux.
Risks to discuss with your surgeon
Rebuilding the digestive tract can lead to serious complications that affect recovery and quality of life and can be life-threatening. After removal of the upper part of the stomach, the normal barrier against reflux is lost. Possible problems include inflammation of the food pipe from reflux, slow emptying of the remaining stomach and narrowing at the surgical join. Anti-reflux reconstruction techniques aim to reduce reflux while retaining some stomach function.
Recovery and follow-up care
Nutrition is part of care both around the operation and over the longer term. Nutritional health before surgery is linked to complications and cancer-related survival. Starting oral feeding early rather than later can shorten the hospital stay without increasing complications. After total gastrectomy, routine vitamin B12 supplementation is recommended. High-dose oral treatment and injections can both treat vitamin B12 deficiency. After surgery that preserves part of the stomach, follow-up also assesses digestion, nutrition and quality of life. Cancer follow-up is a separate part of care and can be tailored to the cancer stage, with scans playing a central role.
