Decision guide
Rectal cancer surgery often uses total mesorectal excision, or TME, which removes the rectal cancer together with the surrounding mesorectal tissue.

Rectal cancer surgery often uses total mesorectal excision, or TME, which removes the rectal cancer together with the surrounding mesorectal tissue. The surgical approach and the way the bowel is reconnected are separate choices. TME can be performed with laparoscopic instruments, with robotic assistance, or using a transanal approach through the anus. Reconnecting the bowel is called an anastomosis; research compares both stitched and stapled connections. A 2020 international consensus recommends referral to expert centres for complex cases likely to benefit from transanal TME.
TME is an established operation for cancer in the middle and lower rectum. For selected patients, removing only the tumour locally after radiotherapy may offer an organ-preserving alternative. A 2026 review studied patients with cancer up to clinical stage T3 and/or a favourable response to treatment before surgery. This is a selected group, not everyone with rectal cancer.
A different situation arises when cancer extends beyond the usual TME surgical boundaries or has returned locally. A 2023 consensus describes more extensive surgery, called pelvic exenteration, for these situations and stresses assessment by a team of specialists to balance complete cancer removal against loss of organ function. This is not interchangeable with routine TME.
A 2026 review of four randomised trials included 1,952 patients with middle or lower rectal cancer, with tumours no more than 10 cm from the anal opening. Compared with laparoscopy, robotic TME had lower odds of needing a switch to open surgery during the operation: the pooled odds ratio was 0.41, with a 95% confidence interval of 0.21 to 0.79. This compares the odds between approaches; it is not a patient’s absolute chance of needing open surgery. Complications during surgery and early after surgery did not differ clearly.
Two of those trials reported cancer outcomes at three years. The findings favoured robotic surgery for cancer returning in the pelvic area and for remaining alive without disease, while overall survival did not differ clearly; few recurrence events mean these findings need cautious interpretation.
For the organ-preserving alternative, a separate 2026 review included six studies with 679 selected patients: 366 had local tumour removal plus radiotherapy and 313 had TME. Local removal plus radiotherapy was associated with fewer complications and a shorter hospital stay. These findings support discussing this option for selected patients rather than treating it as a replacement for TME in all rectal cancers.
Risks relevant to TME include leakage where the bowel is reconnected and problems with urination or sexual function. A 2021 consensus focuses on preserving a tissue layer called Denonvilliers’ fascia to protect urinary and sexual function during TME. A 2026 review also compared methods of reconnecting the bowel, but its low-certainty evidence does not establish one technique as the safest choice for everyone.
The extent of surgery matters. In patients with locally advanced rectal cancer treated with chemotherapy and radiotherapy before TME, a 2026 review associated additional removal of lymph nodes from the side of the pelvis with more urinary and sexual dysfunction. The more extensive pelvic exenteration operation carries risks of serious complications, permanent loss of organ function and reduced quality of life. Those risks concern these specific treatment situations, not every rectal resection.
Preparation before surgery and monitoring afterwards are different parts of care. For minimally invasive rectal surgery, a 2023 guideline conditionally recommends mechanical bowel cleansing together with oral antibiotics and an enema. A conditional recommendation allows for individual circumstances rather than setting one preparation plan for everyone.
After colorectal cancer surgery intended to cure the cancer, follow-up includes attention to new cancers in the bowel and, for rectal cancer, cancer returning locally. A 2016 surveillance guideline addresses colonoscopy, an examination inside the bowel with a camera, as well as specific checks for local recurrence after rectal cancer surgery.

After the diseased portion of the intestine or rectum is removed, intestinal continuity is restored by creating a colorectal anastomosis. To protect this surgical connection during the critical healing phase, the WAFI technique uses a controlled tube-based ileostomy diversion system. This system temporarily diverts intestinal contents away from the anastomosis, helping reduce the risk of complications while maintaining bowel continuity. Unlike conventional diversion techniques that require the creation of a stoma and a second operation for closure, the WAFI system allows temporary diversion through a tube-based mechanism that can be removed once healing is complete. The procedure can be performed using laparoscopic or robot-assisted minimally invasive colorectal surgery, allowing precise oncologic resection while minimizing surgical trauma and supporting faster postoperative recovery. This specialized surgical approach is exclusively performed at Anadolu Medical Center. LAPAROSCOPIC LOW ANTERIOR RESECTION WITH THE WAFI PROCEDURE FOR RECTAL AND COLORECTAL CANCER – by Prof. Vafi Atalay Procedure Includes Low anterior resection WAFI procedure with full diversion tube ileostomy Laparoscopic minimally invasive colorectal surgery Hospital Stay Average 14 days hospitalization (including 1 day intensive care unit stay) Clinical Information Standard pathological examination Postoperative monitoring in the intensive care unit |
