How surgery works and how approaches differ
Bladder removal with reconstruction combines removal of the bladder with a new route for urine, called urinary diversion. One reconstruction described after bladder removal is an ileal neobladder, a replacement reservoir made from part of the small intestine.
When surgery may help and what alternatives to discuss
Bladder removal with a neobladder has been used for bladder cancer that has grown into the bladder muscle. Treatment differs for high-risk cancer that has not grown into that muscle: BCG, a treatment placed inside the bladder, is a standard treatment in this setting. Cancer that does not respond to BCG is unlikely to benefit from more BCG.
Expected outcomes and limits of the evidence
A 78-year-old man who had previously undergone bladder removal with an ileal neobladder needed open surgery to remove two large stones after 18 years without urological follow-up. During the 12 months after stone removal, his symptoms resolved, imaging showed no stones and kidney function remained stable. This single-patient result concerns treatment of a late complication, not the likelihood of cancer control or successful reconstruction after the original operation.
Risks to discuss with your surgeon
Stones can develop in a neobladder and may require another operation. In the man treated after a long gap in follow-up, the stones were linked to a chronic bacterial infection. Vaginal complications can also occur after bladder removal: a 71-year-old woman developed a bulge caused by bowel pushing into the vaginal area 17 months after robot-assisted surgery for bladder cancer. She underwent reconstructive surgery, with no recurrence of the bulge reported six months later.

Recovery and follow-up care
Long-term care of a neobladder includes monitoring for stones, infection and changes in kidney function. After surgery to remove large neobladder stones, one man had follow-up visits at one, three, six and twelve months, including imaging and checks of kidney function. His earlier break in care began after he moved away, highlighting the importance of continuity of urological follow-up when returning home or relocating.
