How surgery works and how approaches differ

Colectomy means surgery to remove large bowel tissue. For cancer, the surgical plan defines how much bowel is removed, the tissue planes followed and the blood vessels divided. These details matter: colectomy is not one identical operation for every patient. In right-sided colon cancer, one choice is between standard right hemicolectomy and complete mesocolic excision. Complete mesocolic excision follows defined tissue planes to remove the bowel with its surrounding supporting tissue intact, with dissection around major blood vessels and removal of lymph-node-bearing tissue. This is a choice about the extent and precision of removal, not simply whether the surgeon uses robotic or laparoscopic instruments.

When surgery may help and what alternatives to discuss

For most patients having surgery for right-sided colon cancer, standard high-quality right hemicolectomy remains the default approach. Complete mesocolic excision may be considered for physically fit patients with high-risk or anatomically complex disease in experienced surgical units. It is not a routine requirement for every right-sided cancer.

Other diagnoses raise different choices. For people with Lynch syndrome and colorectal cancer, removing a bowel segment rather than a larger extent of colon involves a different balance of later cancer risk. For people with ulcerative colitis and abnormal precancerous tissue that cannot be managed endoscopically, removal of both the colon and rectum is the standard approach described in the evidence. More limited removal has been studied in patients with higher surgical risk, but low-quality comparisons do not establish that these options are equivalent. These findings apply to those specific conditions, not to every person considering colectomy.

Sources (3)

Expected outcomes and limits of the evidence

Removing more tissue does not automatically mean a proven improvement in survival. For right-sided colon cancer, a comparison covering 13 studies and 3,743 patients reported better overall survival and survival without disease at three years after minimally invasive complete mesocolic excision than after conventional right hemicolectomy. However, the 2026 clinical position statement concludes that a definite survival advantage remains uncertain and does not support routine complete mesocolic excision for everyone.

In a separate comparison of 14 studies involving 2,303 people with Lynch syndrome and colorectal cancer, 1,724 had segmental removal and 579 had more extensive removal. Segmental removal was associated with more new colorectal cancers later, but no statistically significant difference in overall survival was found at five or ten years. This does not prove that the operations are equivalent, and these results are not a personal survival forecast.

Sources (3)

Risks to discuss with your surgeon

Complications studied after right colectomy include bleeding or leakage at the bowel join, intestinal blockage, wound infection and delayed return of bowel movement. Complete mesocolic excision is technically demanding, and dissection around central blood vessels may add procedural risk; structured training and careful planning are important.

Cancer extent also matters when considering a small-incision approach. In locally advanced T4 colon cancer, a review of retrospective studies found an association between laparoscopic surgery and more later cancer spread to the lining of the abdomen than after open surgery. The evidence was low quality, so this association is not proof that laparoscopy caused the difference. It supports caution for this particular cancer group rather than a conclusion that open surgery is better for all patients.

Recovery and follow-up care

Recovery care is part of the treatment, not just the operation itself. Enhanced recovery nursing programmes have been studied after laparoscopic colectomy. Across 14 studies involving 1,991 patients, these programmes were associated with shorter hospital stays and earlier passage of gas after surgery, a sign of returning bowel function. The size of these benefits varied considerably between studies.

Same-day discharge after elective colectomy is a specific care pathway, not a promise about how quickly everyone can go home. Quality standards for this pathway cover preparation before admission, anaesthesia, surgery, monitoring in hospital and monitoring at home. A discussion about early discharge can therefore include how monitoring will continue once the patient leaves hospital.

After the diseased portion of the intestine or rectum is removed, intestinal continuity is restored by creating a colorectal anastomosis. To protect this surgical connection during the critical healing phase, the WAFI technique uses a controlled tube-based ileostomy diversion system. This system temporarily diverts intestinal contents away from the anastomosis, helping reduce the risk of complications while maintaining bowel continuity. Unlike conventional diversion techniques that require the creation of a stoma and a second operation for closure, the WAFI system allows temporary diversion through a tube-based mechanism that can be removed once healing is complete. The procedure can be performed using laparoscopic or robot-assisted minimally invasive colorectal surgery, allowing precise oncologic resection while minimizing surgical trauma and supporting faster postoperative recovery. This specialized surgical approach is exclusively performed at Anadolu Medical Center.
