How surgery works and how approaches differ
Hyperthermic intraperitoneal chemotherapy (HIPEC) delivers heated chemotherapy directly into the abdominal cavity. It is studied alongside cytoreductive surgery, an operation to remove tumour tissue. The choice is not simply whether to have surgery, but whether adding HIPEC is appropriate for the particular cancer and treatment stage. Drug regimens, temperature settings and timing vary between approaches.

When surgery may help and what alternatives to discuss
The role of HIPEC differs by cancer type. A 2025 comparison of national guidelines found the strongest agreement for combining it with surgery in pseudomyxoma peritonei and peritoneal mesothelioma, diseases affecting the lining of the abdomen. Recommendations were divided for colorectal cancer and newly diagnosed ovarian cancer; many guidelines advised against HIPEC for recurrent ovarian cancer and gastric cancer.
For advanced ovarian cancer, one studied choice is tumour-removing surgery with or without HIPEC after chemotherapy given before the operation. A 2022 review of randomized trials found a five-year survival benefit in this treatment setting, but not for surgery without prior chemotherapy or for recurrent disease. This distinction matters: evidence for adding HIPEC at one stage of treatment does not establish its benefit at another. Gastric cancer findings also differ: a 2022 review combining randomized and nonrandomized studies reported early survival benefits, while another 2022 review restricted to randomized trials found no significant benefit at one, two or three years, but a benefit at five years.

Expected outcomes and limits of the evidence
For ovarian cancer, a 2022 review included six randomized trials with 737 patients. In the subgroup of 393 patients with newly diagnosed advanced disease who received chemotherapy before surgery, adding HIPEC improved survival at five years. The recurrent-cancer subgroup, involving 218 patients, showed no significant five-year survival advantage. Older 2019 reviews had reported longer overall survival in recurrent ovarian cancer, whereas randomized-trial reviews in 2022 and 2025 did not establish that benefit.
For endometrial cancer that had spread to the abdominal lining, a 2024 review compared 21 studies of surgery alone, covering 1,116 patients, with 10 studies of surgery plus HIPEC, covering 152 patients. Pooled survival at one year was 82.28% with HIPEC versus 64.68% without it; at two years it was 56.07% versus 36.95%. At five years, the difference of 21.88% versus 16.45% was not statistically significant. These were comparisons across separate collections of studies, not a single randomized head-to-head trial; differences were also less statistically convincing when the analysis was restricted to studies published after 2010. These figures describe that specific cancer population, not the expected outcome for every person having HIPEC.

Risks to discuss with your surgeon
Potential problems discussed in ovarian cancer research include kidney toxicity, injury to the stomach or intestines, and delayed recovery. Evidence about severe complications has changed: a 2022 review of randomized ovarian cancer trials found no statistically significant difference in severe adverse events between treatment groups, whereas a 2025 review of eight randomized trials involving 1,259 patients found more severe adverse events with HIPEC. The survival findings therefore need to be considered alongside the additional treatment burden, rather than treating HIPEC as a risk-free addition to surgery.

Recovery and follow-up care
HIPEC can add to the time spent in hospital. In a 2025 review of eight randomized ovarian cancer trials involving 1,259 patients, surgery with HIPEC increased the initial hospital stay by an average of 1.49 days compared with surgery without HIPEC; this is an additional stay, not the total recovery time.
Nutrition is part of preparation and recovery. A 2023 review of 25 studies of surgery with HIPEC found that poor nutritional status before treatment was associated with postoperative infections and longer hospital stays in several studies. Nutritional assessment can include a clinical assessment of nourishment and measurements of muscle loss, rather than relying on body weight alone.
