How surgery works and how approaches differ
Transurethral resection of the prostate (TURP) is an operation used to relieve blockage of urine flow caused by benign prostatic hyperplasia, or non-cancerous prostate enlargement. The aim is to improve urination. Research includes a form called monopolar TURP, as well as comparisons with laser operations for benign enlargement; these are different treatment techniques, not interchangeable names for the same procedure.

When surgery may help and what alternatives to discuss
The evidence discussed here concerns urinary obstruction from benign prostate enlargement. Holmium laser enucleation of the prostate, called HoLEP, is a surgical alternative to TURP for this condition. A 2023 review of randomized trials found that HoLEP involved a longer operation but shorter catheter use and hospital stay. It also found better urinary flow at 12 and 24 months and better urinary symptom scores at 12 months with HoLEP.
Bladder muscle strength also matters. In a 2025 study of 163 patients with benign enlargement and an underactive bladder muscle, results two weeks after catheter removal varied with the strength of the bladder’s contraction. Stronger contractions were associated with greater benefit from TURP. These findings concern urinary function, not prostate cancer control.

Expected outcomes and limits of the evidence
A 2025 observational study followed 518 men aged 50 or older who had monopolar TURP for urinary obstruction caused by benign prostate enlargement at two hospitals in Pakistan. At 12 months, the average urinary symptom score fell from 23.74 to 7.82 points. Average maximum urinary flow increased from 7.89 to 17.50 millilitres per second, while the average amount of urine remaining in the bladder after urination fell from 126.40 to 30.10 millilitres.
These are group averages from an observational study, not an individual prediction or evidence of cancer cure; the findings apply to monopolar TURP for benign obstruction.
Risks to discuss with your surgeon
In the same 518-patient study, which assessed outcomes through 12 months, early complications included blood in the urine in 42 patients (8.11%) and urinary tract infection in 61 (11.78%). Later complications included narrowing of the urine passage in 3.67% and narrowing at the bladder outlet in 2.12%.
The 2023 comparison of HoLEP and TURP found lower risks of blood transfusion, low blood sodium and narrowing of the urine passage with HoLEP, but a higher risk of painful urination. A separate 2026 randomized trial in 121 patients with benign obstruction and prostates larger than 90 millilitres compared diode laser treatment in 61 patients with TURP in 60. At 6 and 12 months, ejaculation into the bladder rather than outward was less frequent after diode laser treatment; urinary symptom and flow measurements did not differ significantly between groups.

Recovery and follow-up care
Recovery involves catheter care and assessment of urination after the catheter is removed. In the 2023 review, HoLEP was associated with a shorter period of catheter use, bladder rinsing and hospitalization than TURP. The 2025 study of 518 men assessed urinary symptoms, urine flow and urine remaining in the bladder at 1, 6 and 12 months after monopolar TURP. These were research follow-up visits, rather than a personalized schedule.
Laboratory examination of tissue removed during TURP can also identify an unsuspected prostate cancer. A 2025 study of tissue specimens from 218 patients at a single centre in rural Central India highlighted the importance of this examination in a setting with limited screening.
