How surgery works and how approaches differ
Hysterectomy is surgery to remove the uterus. For early-stage cervical cancer, choices about radical hysterectomy include the surgical route and whether a nerve-sparing technique is appropriate. Nerve-sparing surgery aims to protect pelvic nerves involved in bladder, bowel and sexual function. Open surgery and minimally invasive surgery are not interchangeable choices when cancer control is the priority.

When surgery may help and what alternatives to discuss
Radical hysterectomy is a surgical treatment used for early-stage cervical cancer. Before choosing a minimally invasive approach, discuss the cancer-control evidence and the treating team’s results with a gynecologic oncologist. The comparison here concerns different ways of performing radical hysterectomy for cervical cancer, rather than treatment of non-cancerous uterine conditions.

Expected outcomes and limits of the evidence
For cervical cancer, the evidence favors open radical hysterectomy over minimally invasive radical hysterectomy for remaining free of disease. The findings on overall survival are less consistent: a 2019 position statement reported lower overall survival after minimally invasive surgery in a randomized trial, whereas a 2026 review of 15 studies found no statistically significant overall-survival difference. That later review found better disease-free survival at five years after open surgery. A finding of no statistically significant difference does not establish that the two approaches are equivalent.

Risks to discuss with your surgeon
Radical hysterectomy for cervical cancer can damage pelvic nerves and lead to difficulties with urination, bowel movements or sexual function. A 2026 review of 10 trials involving 1,247 patients undergoing abdominal radical hysterectomy estimated bladder injury during the operation at 0.2% and injury to a ureter, a tube carrying urine from a kidney to the bladder, at 0.3%. These are pooled estimates across the review, not a prediction for an individual patient. Problems after surgery can also include difficulty emptying the bladder, urine leakage, blockage of urine flow or an abnormal connection between the urinary tract and vagina. Studies differed substantially in how they defined and measured these later problems.
Recovery and follow-up care
After robotic hysterectomy, some patients go home on the day of surgery and others stay in hospital. Across 10 studies involving 3,200 patients, 1,801 went home the same day and 1,399 were admitted. Additional bowel or lymph-node surgery, conversion to open surgery, anemia and some existing health conditions were associated with a lower likelihood of same-day discharge. These findings concern robotic hysterectomy and do not describe recovery after every type of hysterectomy. After hysterectomy for cervical cancer or precancerous cervical changes, abnormal cells can still develop in the vagina.
